Dear Family Member, Responsible Party or Employee,

Evergreen Place - Champaign has received confirmation that we have had the following in our building:

X additional confirmed positive for staff member(s)

In accordance with the Illinois Department of Public Health (IDPH) and our local health department, we are reporting this information to you in written format and in line with the latest communication guidance. We aim to keep you informed as new information that affects our residents and staff materializes in our building regarding COVID-19. Our staff will also verbally notify you.

Both the IDPH and the local health departments have been informed of our confirmed positives. We work with both agencies to prevent and reduce the risk of the virus in accordance with the CDC guidelines. Upon confirmation of the above positive test results, we follow a specific COVID-19 plan and quarantine and isolate residents who have tested positive to their individual apartments. Additionally, we continue to follow our stringent infection prevention measures such as universal masking for our staff, masking requirements for residents while outside of their apartment, the appropriate use of personal protective equipment (PPE), hand hygiene protocols, social distancing, and multiple screenings for temperature/symptoms of staff during each shift.

This is a time of challenge with visitor restrictions set forth by the IDPH in early March and changing guidelines to accommodate the latest developments affected by the virus. We thank you for your extreme patience and understanding during this time.

Sincerely,

Jamie Beck

Director

Family and Friends of Evergreen Place Champaign:

The team at Evergreen Place and I have found much to be thankful for: our residents, families, and each other. Together, we have endured many of the challenges during the world’s pandemic; I am grateful for your continued trust, patience, and understanding in our community.

We continue to follow the guidance of the Champaign County Health Department, and in coordination, with the recommendations of the CDC and IDPH our nurses have been conducting weekly COVID-19 mass testing; this week we are pleased to report negative test results for all!

We remain in Phase 1 for the health and safety of our residents and team members. As a reminder, Phase 1 includes the following restrictions:

  • Indoor visitation is prohibited, with the exception of end of life compassionate care visits, and is considered on a case-by-case basis
  • Outdoor visitation may occur; please reach out to us to schedule (217) 530-4300
  • Stylists/Salons are not permitted
  • Virtual visitation is available to all residents via several video conferencing platforms

Our celebration looks much different than in years past, however, there is one thing that remains the same… our delightful Thanksgiving spread. Our residents and the team will enjoy quite the feast: turkey and roast beef, mashed potatoes, and gravy, candied sweet potatoes, green bean casserole, corn casserole, buttered noodles, dressing, and last but certainly not least, pumpkin and pecan pies!

Because we have paused all communal meals and small group activities, the need for emotional, spiritual, and social support is great. We encourage you to share opportunities with your family, friends, church groups, volunteers, neighbors: please give your loved ones who reside at Evergreen Place a call!  We are also happy to deliver mail, pictures, letters, email messages, and cards to brighten our residents’ day.  Additionally, feel free to call the front desk and arrange a virtual visit via FaceTime, Skype, or Google Duo, and yes, window visits… the need to help our residents and families stay connected is great, now more than ever as Thanksgiving and the holidays near.

Please continue to visit this landing page for regular communication and to keep informed. Happy Thanksgiving to everyone; it’s an honor to serve you and your families.

Warmly,

Jamie Beck, LPN

Family and friends of Evergreen Place Champaign,

Thank you for your patience; it is my goal to share updates with you as quickly as they are occurring.

We continue to follow the guidance of the Champaign County Health Department, and in coordination with the recommendations of the CDC, IDPH, and CMS, our nurses have conducting COVID-19 mass testing today for our resident residents as well as team members. As of today, our residents have not experienced symptoms commonly associated with COVID-19. We anticipate having results of our mass testing within 72 hours; however, it could take longer. We will update residents and families when test results are received.

Due to our recent COVID-19 positive employee cases, our team members and residents have a unique opportunity to voluntarily participate in an Ely Lilly clinical trial. For further information and questions, please contact Alex, Patient Educator for Ely Lilly at (813) 523-5330.

At this time, we continue to follow the direction of the health department and IDPH; it is their continued recommendation to ‘shelter in place’ in an attempt to control the spread of the virus.  For our residents, this means staying in their apartment. We will continue to screen temperatures of residents throughout the day, as well as employees at the start of shift and mid-shift. Infection control and prevention procedures are common and routine in our community, however, heightened during this time.

Because we have paused all communal meals and small group activities, the need for emotional, spiritual, and social support is great. We encourage you to spread the word to your family, friends, church groups, volunteers, neighbors: please give your loved ones who reside at Evergreen Place a call!  We are also happy to deliver mail, pictures, letters, email messages, and cards to brighten our residents’ day. Additionally, feel free to call the front desk and arrange a virtual visit via FaceTime, Skype or Google Duo… we need to help our residents and families connected.

Please continue to visit this landing page for regular communication and to keep informed. Thank you for your continued support, well wishes, and prayers.

Dear Family and Friends:

Regrettably, we need to inform our residents and families that an Evergreen Place Champaign employee has tested positive for COVID-19. Because of this and other variables, the Champaign County Health Department's direction is to remain in Phase 1 for the health and safety of our residents and team members.

As a reminder, Phase 1 includes the following restrictions:

· Indoor visitation is prohibited, with the exception of end of life compassionate care visits and is considered on a case-by-case basis

· Outdoor visitation may occur; please reach out to us to schedule (217) 530-4300

· Stylists/Salons are not permitted

· Virtual visitation is available to all residents via several video conferencing platforms

Per the guidance of the Champaign County Health Department, and in coordination with the recommendations of the CDC, IDPH, and CMS, our nurses will continue conducting weekly COVID-19 mass testing on Mondays for both our residents and team members until we have two consecutive weeks of negative results. We anticipate having results within 72 hours; however, it could take longer. We will update residents and families when test results are received.

We are also happy to deliver mail, pictures, letters, email messages, and cards to brighten our residents’ day. Additionally, feel free to call the front desk and arrange a virtual visit via FaceTime, Skype, or Google Duo… we need to help our residents and families connected.

Thank you all for your support and understanding. Please use this page to keep informed; I am also happy to take your calls and questions.

Very thankful for your continued trust,

Jamie Beck, LPN

October 14th, 2020

Greetings residents, families, and friends of Evergreen Place Champaign,

It is an ever-changing healthcare climate as the Coronavirus Disease 2019 (COVID-19) spread continues in our cities, across the country, and the world. Thank you for your patience, as it is our priority to directly care for your loved ones at Evergreen Place Champaign. We do understand the importance of timely communication and intend to give updates regularly regarding COVID-19 and the status of our community.

We can share with you that our community has an outbreak of COVID-19. Privacy laws prevent us from sharing details with you, for example, names of residents, areas of the building, apartment numbers, etc. Thank you for your understanding as we preserve our residents’ and team members’ privacy and dignity.

We can also share that we are in constant communication with the Champaign County Health Department, following their guidance and ensuring the proper protocols are being followed to minimize the additional risk of exposure to our residents and our team members. We are regularly screening our residents' temperatures throughout the day and employees at the start and middle of every shift. Infection control and prevention procedures are common and routine in our community, however, heightened during this time. As an added measure to control the virus's spread, residents have been ‘sheltering in place.’ For our residents, this means staying in their apartment. We ask for your assistance in emphasizing the importance of this measure with your loved one through your calls, virtual visits, and emails. They truly love hearing from you!

Please continue to use this page for updates; however, I am happy to take calls your calls and questions if urgent, as I will be focusing on continuing to lead our team and residents through this uncharted territory. I realize how startling this communication may be; rest assured, we have never worked harder to protect your loved ones. We love them too.

Sincerely,

Jamie Beck, LPN
